A unit fresh from the factory failed its load test.

Found
A newly delivered unit, fresh from the manufacturer, showed a hydraulic oil leak at the outrigger cylinder during load testing.
Action
We recommended taking it out of service until repaired.
Result
Repaired within a week. Passed the re-test and cleared for safe operation.

30 years in service. Never inspected. Half the safety it needed.

In service for about 30 years and never inspected. UT thickness gauging showed the shell plate had thinned far below its original thickness. Our computation put its factor of safety at about 2.5, half the required 5.
Action
We recommended derating the tank. Since operations could not run at a lower pressure, the client chose to replace it.
Result
An unsafe vessel was retired before it could fail. The client told us they never suspected it, because the thinning could not be seen from the outside.

A homemade label more than tripled what the sling could actually lift.

Found
A sling labeled 10 tons was actually a 3-ton sling. Its yellow color and narrower width matched a 3-ton sling under the standard color code, where 10-ton slings are orange and much wider. The client had labeled it themselves, assuming it matched their equipment.
Action
We held a short learning session with their team on how to verify sling capacity using the manufacturer’s tag, color, and size.
Result
The sling, still in good condition, was correctly identified as 3 tons before it was ever used on a heavy lift. The client can now spot mislabeled slings on their own.
Wrong reeving quietly cut the crane’s safe capacity in half.

Found
During our overhead crane safe operation training, we found operators had no formal training and skipped pre-use inspection. The hoist was reeved with a 2-part line, while the manual called for a 4-part line, cutting the wire rope’s capacity to about half of the crane’s 5-ton rating.
Action
We trained the team on safe operation and daily pre-use inspection, and walked them through the reeving issue on their own crane.
Result
Caught before a near-capacity lift. The client thanked us, saying they now understand why proper training matters.

Badly damaged ropes, hidden in plain sight since installation.

An elevator never inspected since installation. Its hoist wire ropes had many broken wires, with damage already at a severe stage.
Action
We recommended stopping operation and replacing the wire ropes.
Result
Ropes replaced within two weeks. Passed re-inspection. The client said they had no idea how damaged the ropes were until the inspection.

During an overhead crane inspection training at a manufacturing plant, a participant said overloading on every lift was fine, since the crane has a factor of safety of 5.
Action
We explained that this is a common myth. The factor of safety of the hoist does not guarantee the same margin for the runway, girders, building structure, and other load-bearing parts, which may be lower, especially if materials were cut to save cost.
Result
The team agreed and committed to stop overloading. They also recognized that the crane and its supporting structure must be properly maintained for safe operation.
Operators hit the upper limit switch on every single lift.

Found
During our overhead and gantry crane operation training, we saw operators routinely hoisting until the upper limit switch tripped, using it to stop every lift.
Action
We explained that the upper limit switch is an emergency backup for when an operator fails to stop in time, not an everyday stop. It should only be tested during pre-use inspection. Tripping it on every lift wears it out and can cause it to fail when it is needed most.
Result
The operators said no one had ever taught them this before, as they thought that was how the switch was meant to be used. They now stop the hoist themselves.

A chain is only as strong as its weakest link.

One worn link, unnoticed for a long time, set the limit for the whole chain.

Found
We found a load chain link that was visibly worn down and needed replacement. It had been in that condition for a long time without anyone noticing.
Action
We showed the damaged link to the client and explained that a chain is only as strong as its weakest link. We trained their team to check every link, 100%, so nothing slips through, and recommended taking the hoist out of service.
Result
The hoist was not used until the load chains were replaced. We returned to inspect it after the replacement.

Meet Mark B. Clave, co-founder and VP for Operations. Mark has spent more than a decade around heavy equipment. He holds a TESDA NC II in heavy equipment operation, and he is an OSHC Authorized Inspector and NDT Level 2 practitioner in UT, PT, and MT. Because he knows how this equipment is operated, he knows exactly where it fails. On site, he runs the inspection and makes sure nothing critical is missed.

Common questions
Does our construction heavy equipment need DOLE testing and certification?
Yes. Under DOLE Department Order No. 13, Section 10, heavy equipment used in construction must be tested and certified by a DOLE-accredited testing organization. BNW IntegriSafe is accredited by DOLE-OSHC for this.
Do boilers and air receivers need a Permit to Operate?
Generally, yes. Under the Occupational Safety and Health Standards (Rule 1160 for boilers and Rule 1170 for unfired pressure vessels such as air receivers), these require inspection and a Permit to Operate from DOLE. We perform the testing and prepare the supporting documents for your application.
What do you need from us to prepare a quote?
The equipment type, capacity, quantity, site location, and your target schedule. Photos of the nameplates help us quote faster and more accurately.
Do you serve sites outside Metro Manila?
Yes. We mobilize nationwide. Travel and mobilization are included in your quotation based on the site location.
What will we receive after the inspection?
A signed report for each item, plus a certificate where applicable. Heavy equipment gets a PASSED sticker, and lifting gear gets a sticker or tag. NDT includes an engineering evaluation, and PTO/CEI work includes as-built plans and drawings. All certifications are signed and sealed by a licensed PME.
